USA PATRIOT Act § 326 · IRS Form W-9 Verification
Under Section 326 of the USA PATRIOT Act and federal custodial clearing regulations, Charles Schwab & Co., Inc. requires your Taxpayer Identification Number to establish and federally insure your account.
Step 1 of 8
Before we begin, please confirm the accounts you asked us to open and transfer. If anything looks off, tell us now — it’s easier to fix here than after submission.
Schwab requires a short financial profile for every new account. Ranges are fine — pick the closest.

Are you interested in cryptocurrency exposure?
Do you have brokerage accounts at other firms?
Because a UGMA/UTMA is opened for a minor, we need the minor's information. You (the guardian) remain the custodian until the age of majority in your state.
Because a Custodial Roth IRA is opened for a minor, we need the minor's information. A Roth IRA requires earned income; please tell us where the minor works.
Are you, your spouse, or any other immediate family members (including parents, in-laws, siblings, or dependents):
Employed by or associated with the securities industry (for example, a sole proprietor, partner, officer, director, or branch manager of a broker-dealer firm) or a financial regulatory agency?
Which type?
Notifications Requirement
Affiliated Account Disclosure Requirement
Since you are employed by or affiliated with a FINRA-regulated broker-dealer or a registered investment advisor (RIA), federal securities regulations require you to obtain prior written approval from your employer’s compliance department before opening or maintaining a personal investment account through an outside advisor and custodian. Your CleverAlpha-managed account is custodied at Charles Schwab & Co., Inc.; both relationships must be disclosed.
Why This Matters
FINRA Rule 3210 helps prevent conflicts of interest and ensures proper oversight of your trading activities. Your employer may also request duplicate trade confirmations and account statements from Schwab (as the carrying firm and custodian of record) to monitor your account.
Send the 3210 Letter Draft
Choose how you would like to handle the FINRA Rule 3210 (formerly 407) letter granting permission for this account.
Does your employer require notification of accounts held elsewhere?
✓ Duplicate trade confirmations and account statements will be routed directly to your employer's compliance department.

Three yes/no questions. Required for FATCA and IRS backup-withholding compliance.

You told your advisor you have an old retirement plan you’d like to roll into your new IRA. Before we finish, we need to confirm a few things about that decision.

You have four options for your old plan
Leave the money in your former employer’s plan (if allowed).
Roll it into your new employer’s plan (if allowed).
Roll it into an IRA (this is what you’re doing today).
Cash it out (taxes and, if under 59½, an additional 10% penalty may apply).
We recommend rolling into an IRA because it consolidates your retirement money in one account you control, gives you a wider set of investment choices than most employer plans, and lets us manage it as part of your overall plan. On the next screen we’ll walk through the specific costs and trade‑offs so you can decide with your eyes open.
A couple of details so we can prepare the paperwork, plus one confirmation before we proceed.
One thing the IRS treats differently after you roll: Required Minimum Distributions (RMDs).
What changes about RMDs
If you'’re still working for the sponsor of your old plan, that plan may let you delay RMDs on it until you retire. IRAs don'’t offer that delay — RMDs kick in at the SECURE Act age (currently 73, rising to 75 in 2033) regardless of your work status.
If you roll into an IRA, that money follows the IRA RMD rules. Once you'’re at RMD age, you'’ll take an annual distribution from the combined IRA balance.
Missing an RMD is expensive: the IRS penalty is 25% of the shortfall (10% if corrected within 2 years). We'’ll set up reminders on our end once you'’re within a few years of your first RMD.
If you’re still actively working for the sponsor of the plan you’re rolling and staying in the plan would delay your RMDs, we can pause this. Otherwise the IRA path is what most clients pick.
Does keeping the money in your old plan actually delay an RMD you’d otherwise have to take?
The honest cost picture. Neither side is universally cheaper — here'’s what tends to be true.
Old employer plan
Institutional share classes with fund fees often between 0.02%–0.30% per year. Plan admin fee is usually flat, sometimes bundled, and varies widely by employer.
New IRA with us
You pay the underlying ETF/fund fee (typically 0.03%–0.20%) plus our advisory fee. No plan admin fee. You get a much wider investment menu and can hold cash, individual securities, and alternatives that most 401(k)s can'’t offer.
When your old plan is likely cheaper: very-low-cost institutional funds you already own, no plan admin fee, and you'’re happy with the menu. Rolling in that case may raise your total cost.
When the IRA is usually the right call: the plan menu is limited or expensive, you'’re juggling accounts across former employers, or you want the account to be part of a single managed plan instead of standalone.
Link your bank account via secure ACH to fund your initial deposit.
